Reporting occurrences: simple and important!
Reporting occurrences plays a crucial role in improving our safety. Whether it is an accident, an incident, or a situation that could have led to something serious, your report matters and fuels the Safety Management System (SMS). Every report we receive from colleagues helps the organisation move forward, helps us learn from incidents and become even safer.
Don't worry about confidentiality; all reports are kept strictly confidential by the KLM (SCO) and E&M (S&Q) Safety organisation. If necessary, you can even make a report anonymously. KLM is committed to an open reporting culture. The aim is to become safer, stay safe and not to assign blame. We call the environment in which we do that ‘Just Culture’. What happens to your report? Your Maintenance Safety Report (MSR) are received by our S&Q colleagues in Building 404, other types of reports are sent to the SCO in Building 201. The report is assessed and classified. Using an ERC tool, each report is given the appropriate attention, based on severity and urgency. All information about your report is processed in a file, which is stored in the Safety IT management system (Q-Pulse).
Feedback If you've given your name on a MSR, you will always receive feedback. Some reports are mandatorily passed on to the authorities (IL&T) and the manufacturer concerned. These are called Mandatory Occurrence Reports (MORs). Reports that we pass on voluntarily are called Voluntary Occurrence Reports (VORs). When these reports are passed on, they do not include the names of reporters or colleagues.
Making trends visible Your report helps to analyse trends. The data from Q-pulse are used to make trends visible, by looking at the bigger picture. These trends are discussed monthly with the various units. This can lead to actions such as in-depth research, broad communication or even a risk analysis. We call this a SIRA (Safety Issue & Risk Analyses).
When in doubt make a report So.... if you see a dangerous situation at your workplace, on the road or somewhere else, report it immediately, clearly and with as much detail as possible! This is very important to determine proper classification and follow-up. Even if you are in doubt, make a report anyway. Our specialists will then determine whether it should be reported in accordance with the applicable guidelines.
